Accordingly, around 12 o’clock on April 22, after lunch break, Mr. Le Manh Cuong, Head of the production shift of the finished product workshop, Mr. Nong Van Tuan and 8 other people were all workers working for the Cement Factory. Yen Bai transports materials inside cement mill No. 3 (ball mill) to repair and replace lining plates. But Mr. Cuong did not cut off the power (cut off the power) according to the repair process.

At 12:49 on the same day, Ms. Tinh (an employee of the Central Control Room) observed through the camera screen and discovered Mr. Tuan and a number of people sitting on crusher No. 3 repairing the machine. Ms. Tinh saw that there was still power to crusher No. 3, so she called Mr. Tuan to inform him that the machine still had power and requested to cut off the power.

At 1:13 p.m., Mr. Cuong called Mr. Vu Xuan Toan (Head of the production shift of the raw kiln workshop) to ask the person on duty in the Hot Wind room, 6KV Power Distribution Station to cut off the power source of crusher No. 3.

After that, Mr. Toan continued to notify Mr. Nguyen Anh Tuan (worker on duty in the Hot Wind room) via walkie-talkie to cut off the crusher’s power source.

At this time, Mr. Tuan went to the electric circuit breaker and used the crank to pull it, but it didn’t work, so he called Mr. Toan and told him to find an electrician to fix it. Mr. Toan then disconnected the circuit breaker from the control center and went to find an electrician.

At this time, Tran Manh Hung (material weighing employee) was there, took a piece of broom handle made of peeled wood about 80cm long that was available there and poked it into the electrical relay on the cutting machine, starting crusher number 3. leading to the rotary crusher.

The accident killed 7 people inside the crusher. 3 people sitting on the crusher were thrown to the ground and injured, and were taken to the emergency room and treated at Yen Bai Provincial General Hospital.
