(born 1995) and The other 8 people are all workers at Yen Bai Cement Factory, transporting materials inside cement crusher No. 3 to repair and replace lining plates.
At this time, Mr. C. did not cut off the power according to the repair process.
At 12:49 on the same day, Ms. Nguyen Thi Tinh (born 1984, residing in Yen Binh district), an employee of the Central Control Room, observed through the camera screen and discovered Mr. T. and a number of people sitting on crusher No. 3. , you should know that these workers are repairing the crusher.
The moment the crusher suddenly rotated caused 7 Yen Bai cement workers to die.
Next, Ms. Tinh saw through the computer screen that there was still power to crusher No. 3, so she called Mr. T. to inform him that the machine still had power and suggested cutting off the power.
A while later, seeing that crusher No. 3 still had power, Ms. Tinh contacted Mr. C. via walkie-talkie, saying “Why don’t you cut off the power?” Mr. C. said, “You’ve already cut off the power to the resistor”.
At 1:00 p.m. on the same day, Mr. C. called Mr. Vu Xuan Toan (born 1979, residing in Yen Binh district), the head of the factory production shift, asking Mr. Toan to notify the person on duty in the hot air room, 6KV power distribution station. to cut off the power source of crusher No. 3.
Mr. Toan then informed via walkie-talkie Mr. Nguyen Anh Tuan (born 1982, residing in Yen Binh district), a worker on duty in the hot air chamber. Then Mr. Tuan went to the electric cutter of crusher No. 3, used the crank to pull the cutter out, but could not pull it.
Afterwards, Mr. Tuan called Mr. Toan over the radio to say it was stuck and could not be pulled and told Mr. Toan to go find an electrician to fix it.
At this time, Tran Manh Hung (born 1980, residing in Yen Bai city), a material weighing employee working there, also heard it, immediately went to the crusher, saw Mr. Toan riding a motorbike into the hot wind chamber, so Hung follow.
Mr. Toan went to the front of the cutter of crusher No. 3 and used the crank to pull the cutter but could not. Mr. Toan disconnected the Aptomat from the control center and went to find an electrician.
At this time, only Hung and Mr. Tuan remained. Hung used the crank to pull the mower but couldn’t. Therefore, Hung took a piece of broom handle about 80cm long that was available there and poked it into the energized relay on the breaker, causing crusher No. 3 to rotate.
At this time, Mr. Tuan heard the sound of the relay closing, and at the same time heard someone say over the radio to cut off the power to crusher number 3, so Mr. Tuan used the broom handle that Hung had just held to close the relay to turn off the power to crusher number 3.
As a result, 7 workers working in the crusher died, 3 people sitting on the crusher were thrown to the ground and injured, and were taken to the emergency room and treated at Yen Bai Provincial General Hospital.
On April 23, Yen Bai Provincial Police issued a decision to prosecute a criminal case of violating labor safety regulations at the Yen Bai Cement Factory under Yen Bai Cement and Mineral Joint Stock Company. prosecuted the defendant and ordered the defendant to be temporarily detained for Tran Manh Hung.
