At 12:49 on the same day, Ms. Nguyen Thi T. (born 1984, residing in Yen Binh district), a central control room employee, observed through the camera screen and discovered Mr. T. and a number of people sitting on the chair. crusher number 3, you should know that these workers are repairing the crusher.

The scene of the accident left 10 people injured

Next, Ms. T. looked at the computer screen and saw that there was still power to crusher No. 3, so she called Mr. T. to inform him that the machine still had power and requested to cut off the power.

A while later, seeing that crusher No. 3 still had power, Ms. T. contacted Mr. C. via walkie-talkie to say “why don’t you cut off the power?” Mr. C. said “you’ve cut off the power to the resistor”.

At about 13:00 on the same day, Mr. C. called Mr. Vu Xuan T. (born 1979, residing in Yen Binh district), the head of the factory production shift, asking Mr. Xuan T. to ask the person on duty in the Hot Wind room. 6KV power distribution station to cut off the power source of crusher No. 3.

Mr. Xuan T. then informed via radio to Mr. Nguyen Anh T. (born 1982, residing in Yen Binh district), a worker on duty in the Hot Wind room. After that, Mr. Anh T. went to the electric cutter of crusher No. 3, used the crank to pull the cutter out, but could not pull it out.

Afterwards, Mr. Anh T. called Mr. Xuan T. over the radio to say it was stuck and could not be pulled and told Mr. Xuan T. to go find an electrician to fix it.

At this time, Tran Manh Hung (born 1980, residing in Yen Bai City), a material weighing employee working there, also heard it, immediately went to the crusher, saw Mr. Xuan T. riding a motorbike into the Hot Wind room, so Hung followed.

Mr. Xuan T. went to the front of the cutter of crusher No. 3 and used the crank to pull the cutter but could not. Mr. Xuan T. disconnected Aptomat from the control center and went looking for it.

At this time, only Hung and Mr. T. remained. Hung used the crank to pull the mower but could not, so Hung took a piece of broom handle about 80 cm long that was available there and poked it into the electrical relay on the mower, starting it. crusher number 3, causing crusher number 3 to rotate.

Mr. T. heard the sound of the relay closing, and at the same time heard someone say over the radio to cut off the power to crusher number 3, so Mr. T. used the broom handle that Hung had just held to close the relay to turn off the power to crusher number 3.

As a result, 7 people inside the crusher died, 3 people sitting on the crusher were thrown to the ground and injured, and were taken to the emergency room and treated at Yen Bai Provincial General Hospital.

Regarding the above incident, on April 23, Yen Bai Provincial Police issued a decision to prosecute a criminal case of violating labor safety regulations at Yen Bai Cement Factory under Cement Joint Stock Company. and Yen Bai Minerals, issued a decision to prosecute the defendant and ordered the defendant to be temporarily detained for Tran Manh Hung.

Currently, the Yen Bai Provincial Police Investigation Agency continues to investigate and clarify the incident.
