Rare aviation incidents show that the majority of accidents occur due to human error, not any other factor.

Aeroflot Flight 593, 1994

During a flight from Moscow to Hong Kong in 1994, an Aeroflot pilot let his children into the cockpit to play with the controls. This accidentally caused the plane to exit autopilot mode, leading to a tragic accident that killed all 75 people on board.

The fateful accident occurred on March 23, 1994, on an Airbus A310. The plane crashed in Siberia.

Then, the cockpit voice recording was discovered, showing that co-pilot Yaroslav Kudrinsky put his two children into the cockpit in the middle of the night – Yana, 12 years old, and Eldar, 15 years old.

Both were allowed to sit in the captain’s chair and play with the controls. The pilot should have turned off this controller because the plane was on autopilot.

But when Eldar held the control column for 30 seconds, the system returned to manual control mode. The captain and pilot then returned to their seats and took control of the plane, but it was too late. The plane crashed into the mountain below, killing everyone.

TransAsia Airways Flight 235, 2015

In February 2015, TransAsia Airways Flight 235 crashed into a highway bridge in Taiwan and then plunged into the Keelung River, killing 43 of the 58 passengers. The incident occurred after the pilot mistakenly turned off the engine.

The plane had just taken off from Songshan Airport, Taipei, when one of the engines lost power and the pilot accidentally turned off the only operating engine by pulling the wrong switch. The pilot was also among those killed.

In February 2015, TransAsia Airways Flight 235 crashed into a highway bridge in Taiwan and then plunged into the Keelung River.

One of the pilot’s last words was: “Oh, pulled it wrong.”

The plane tilted sharply, flew over the Huan Dong Bridge and then crashed into the Keelung River below.

Tuninter 1153, 2009

In March 2009 on Tuninter flight 1153, when the plane ran out of fuel, instead of performing emergency procedures, the captain prayed and then the plane crashed into the sea, killing 16 people.

The two pilots were later sentenced to 10 years in prison for the deaths of 16 passengers.

Captain Shafik Al Gharbi and co-pilot Ali Kebaier Lassoued, were accused of praying instead of performing emergency procedures after the plane ran out of fuel due to a mechanical failure and crashed into the sea.

In the cockpit recording played in court, Mr. Gharbi is heard calling for divine help. There is evidence that the crew tried several times to save the situation but eventually panicked and allowed the accident to occur.

The plane was en route to Bari, Italy after departing from Djerba, Tunisia, and 23 of the 49 passengers on board survived after being rescued from the water.

Collision between KLM and Pan Am aircraft

In March 1977, two Boeing 747s – KLM flight 1736 and Pan Am flight 1736 – crashed into each other at Tenerife airport – killing 583 people. This became the deadliest accident in aviation history

The collision occurred due to a misunderstanding between the KLM flight crew and air traffic control. The Pan Am plane was still on the runway when the KLM plane attempted to take off. Thick fog that day made it impossible for both planes to see each other.

All 248 passengers and crew members on the KLM plane died, along with 326 passengers and nine crew members on the Pan Am. The remaining 54 passengers and seven crew members on the Pan Am plane survived, including the captain.

Airblue Flight 202, Islamabad

This domestic passenger flight crashed on July 28, 2010, near Pakistan’s capital Islamabad, killing all 146 passengers and 6 crew members.

Perhaps the accident could have been avoided if the plane’s co-pilot had protested the captain’s repeated errors, but according to the Pakistan Civil Aviation Authority in November 2011, he was “taken down” by his boss. humiliated” during the flight and “lost self-esteem”.

The captain was accused of having a “harsh, snobbish and dismissive” tone with the co-pilot multiple times during the flight. Then ignored weather warnings from Air Traffic Control – saying “let him say whatever he wants to say”. And he was not opposed by his co-pilot when disaster struck.

Air Florida Flight 90, 1982

The plane crashed into the Potomac River just 30 seconds after takeoff.

On January 13, 1982, the pilots of Air Florida Flight 90, from Washington DC to Florida’s Fort Lauderdale, failed to turn on the plane’s de-icing system despite freezing weather and then crashed. Potomac River, killing 74 passengers.

Additionally, despite taking off in a snowstorm, the crew used an ill-advised method of de-icing, then failed to abort the takeoff even when a snowstorm was detected.

The plane crashed into the Potomac River just 30 seconds after takeoff. Of the 79 people on the plane, only 5 survived and an additional 4 people on the ground died.

Eastern Air Lines Flight 401, 1972

On December 29, 1972, an Eastern Airlines Tristar crashed into the Florida Everglades, killing 101 people on board, including the captain.

The incident occurred because the pilot and co-pilot were distracted by a burned-out light bulb and accidentally hit the lever, causing the plane’s autopilot to turn off. By the time the crew realized they were losing altitude, it was too late. The plane crashed.

75 people survived the accident.
